  • Must provide medical records of medical condition, learning disorder or copy of a previous evaluation with diagnosis.

 School and College Placement
These evaluations are prepared for students that need accommodations in the classroom, extended time, special services or placement into gifted classes. We administer the Wechsler Intelligence Scale for Children and Adult among others.

Social Security Disability
  • A Biopsychosocial evaluation is recommended to organize and gather previous medical conditions, symptoms, dates of accidents, hospitalizations, treatments and medications.    It is essential that the client starts a list of all the current medical conditions and past illnesses. Include the health insurance plans that were used in the past. Organize the list with dates, names of doctors, locations (city and state) and phone numbers.  It is important that you begin to build your medical file. You must provide specific information in order for social security to approve your claim.  
  • Included in the evaluation will be a summary of the limitations and ability to return to work.
